They offer: - Advice about health and care facilities in the whole city area - Advice about funding and support options - Coordination of necessary actions - Networking of people and organizations working in social and care fields Case management helps build a care network in cooperation with external partners (home care, mobile assistance, hospitals, doctors) and provides information about support offers, aid, etc. They also organize seminars and lectures, as well as a weekly lunch meeting for seniors, which is part of the wide range of services offered by the Care and Support Service Center.
