A temporary shelter - that is the motto of SeneCura transitional care. The transitional care is directly connected to the hospital and has 24 beds. The unique offer is aimed primarily at older clients who still need nursing care after being discharged from the hospital. Help for self-help - reactivating transitional care Especially after an operation, an illness or an accident and a related hospital stay, many older people find it almost impossible to cope with everyday life at home on their own. They need nursing and therapeutic help and support for a certain period of time in order to gently find their way back to their old life. Together with the hospitals therapists, the SeneCura team tries to achieve this goal. The clients can decide for themselves how long they want to use this offer. Short-term care to relieve the burden on caring relatives Caring relatives also need a vacation sometimes or are unable to provide care on a continuous basis due to their own illness. For these situations, SeneCura offers transitional care to ensure interim care. If you are interested or have questions, you can contact the SeneCura team directly. SeneCura transitional care was born out of the idea of creating a supplement to the existing, but often over-utilized, care facilities and outpatient services in the Schwaz district. The project was developed in cooperation with the system partners, the municipal association, social insurance providers, BKH Schwaz, home operator SeneCura, state and municipal representatives, outpatient services, family doctors and long-term care facilities. The project was launched in autumn 2011.
