Case Management

Description

Questions that arise after an accident or serious illness are clarified in the course of a detailed consultation by the case manager of the Obergricht social and health district. A coordinated care plan is then developed for the quickest and most comprehensive help possible. Case management collects, coordinates, monitors and evaluates the support and services that clients need in every problem situation. Our services: + Individual advice and support + Advice for people in need of care and their relatives + Assistance with and organization of various support offers (applications) + Networking of professional and voluntary support offers + Support for families in difficult problem situations in order to find a solution together
