Our case manager is the contact person for all residents of our district municipalities for nursing and social matters. We are happy to advise you confidentially, without obligation and free of charge on the following topics: - Support for family caregivers (assistance with care leave, grants toward the costs of substitute care, support for 24-hour care) - Advice (nursing and/or social concerns) - Help with applications (assistance with applying for care allowance, rehab, disability pass, etc.) - Information (criteria for admission to nursing homes, information about short-term and transitional care as well as other care structures) - Support in challenging life situations to help ensure the best possible care - Referral to other care and social institutions such as hospice, day care, rehabilitation, etc.
