After an inpatient stay in our hospital, patients and their relatives should be adequately prepared for care at home by relatives or outpatient care services, 24-hour care or inpatient care facilities. This preparation and ensuring the continuous care of our patients is our core task! The admission and discharge management deals with precisely these questions and thus represents an important relief, not only for the patients concerned, but also for their relatives and all those involved in the care process.
